BACKGROUND: Pulpectomy is used to treat primary teeth with irreversibly inflamed or necrotic pulp tissue. There is, however, a lack of consensus on clinical protocols for teaching pulpectomy at dental schools. AIM: To determine what faculty members at Brazilian dental schools are teaching about pulpectomy in primary teeth. DESIGN: A cross-sectional, observational study was conducted using a 39-question survey available via the Google Forms® platform. A snowball sampling strategy was used for recruitment. Invitations to participate were sent via e-mail, WhatsApp® and Instagram®. The data were analysed descriptively. RESULTS: A total of 177 (58.4%) responses were obtained. Most faculty members recommended the use of Kerr hand files (66.1%), and 1% sodium hypochlorite was the most common choice for the irrigating solution (35.3%). Iodoform pastes were the first choice of faculty members (41.6%), and 60.5% of the faculty members recommended composite resin as the definitive restoration. CONCLUSION: Similarities were found in the teaching of pulpectomy for primary teeth among faculty members at dental schools in Brazil.

OBJECTIVES: This study evaluated the antimicrobial activity, clinical and radiographic outcome of pulpectomy in primary teeth using either 1% sodium hypochlorite (NaOCl) or 2% chlorhexidine (CHX) as irrigants. MATERIALS AND METHODS: A randomized double-blind controlled clinical study in which primary teeth were allocated to 1% NaOCl (n = 20) and 2% CHX (n = 20) groups. Microbiological collections were performed before and after irrigation for agar culture and real-time polymerase chain reaction (qPCR). Clinical and radiographic success was assessed at different times. Data were submitted to descriptive analysis, chi-square, Mann-Whitney, and Wilcoxon tests (p  < .05). RESULTS: For 1% NaOCl, the following clinical and radiographic success rates were observed: 7 days (93%/80%); 30 days, 3 and 6 months (100%). For 2% CHX: 7 days (73%/53%); 30 days (93%); 3 months (100%/93%); 6 months (100%) (p > .05). One percent NaOCl and 2% CHX effectively reduced total microorganisms (p < .05) but not mutans streptococci (p > .05). In qPCR analysis, the solutions promoted a reduction of total bacteria and Streptococcus mutans, and no difference was observed between times and groups (p > .05). CONCLUSIONS: One percent NaOCl and 2% CHX were effective for clinical and radiographic success and antimicrobial activity in primary teeth submitted to pulpectomy. CLINICAL RELEVANCE: Studying the antimicrobial activity and clinical and radiographic outcomes of pulpectomy in primary teeth using NaOCl and CHX as irrigants is clinically relevant because it provides information for optimizing treatment protocols and improving the quality of care for pediatric patients. It contributes to evidence-based practice and can potentially lead to better outcomes, reduced complications, and enhanced patient experiences.

RESUMO

Aggressive periodontitis is a disease that causes severe destruction of periodontal tissues, showing early development and rapid progression in both primary and permanent dentitions. Due to familial aggregation, children of parents with periodontitis are considered to be at higher risk for disease occurrence, which suggests that they should be evaluated and monitored as early as possible. The purpose of this case report is to describe aspects related to early diagnosis of periodontitis in two children and their relationship with the parent's periodontal condition, exploring the familial component as a crucial factor that can lead to an early diagnosis and better clinical management in their offspring.

The treatment of high-risk patients still is a challenge. The understanding and development non-invasive, non-destructive, and non-ionizing techniques, can help to guide the treatment and the diagnosis of primary and recurrent caries. The present study evaluated the behavior of enamel/restoration interface after a cariogenic challenge by Fourier domain optical coherence tomography (FD-OCT), scanning electron microscopy (SEM) examination, and the fluoride release of the different restorative materials. Cavities (1.5 × 0.5 mm) were performed in enamel surface and divided into groups (n = 8): glass ionomer cement (GIC), resin-modified glass ionomer cement (RMGIC), and resin composite (RC). The samples were submitted to pH-cycling, and the solutions analyzed for cumulative fluoride by ion-analyzer. The morphology was analyzed by SEM through replicas. The optical attenuation coefficient (OAC) was calculated through exponential decay from the images generated by FD-OCT. Data were analyzed considering α = 0.05. OAC values increased for all groups after pH-cycling indicating demineralization (p < .05). Considering the remineralizing solution, RMGIC presented higher fluoride release rate, followed by GIC, while RC did not release any fluoride. Yet for the demineralizing solution, RMGIC and GIC released similar fluoride rates, overcoming RC (p < .05). Micrographs revealed no changes on the restorations margins, although enamel detachment was observed for RC and GIC after pH-cycling.

This study evaluated the fluoride (F) release and remineralizing potential of varnishes containing sodium fluoride (5% NaF), 5% NaF with CPP-ACP and 5% NaF with TCP in early caries lesions in primary teeth. To determine the F release at 1, 4, 6, 24, 72, and 168 hr, strips were covered with the varnishes and immersed in purified water (n = 7). The varnishes and purified water (negative control) were applied on enamel blocks with early caries lesions (n = 16). Enamel blocks were stored in artificial saliva and submitted to a pH-cycling. The area of enamel hardness loss (ΔS) was analyzed by microhardness, lesion depth by polarized light microscopy (PLM) and the chemical analysis by Energy-dispersive X-ray spectroscopy. Data were submitted to Shapiro-Wilk, two-way and one-way ANOVA, Tukey and paired t-tests (α = 5%). All varnishes released F, but 5% NaF with CPP-ACP had the highest release at 4, 6, 24, and 72 hr (p < .05) followed by 5% NaF with TCP and 5% NaF. No significant difference in ΔS was observed among varnishes (5% NaF = 4,098.4 ± 1,407.9; 5% NaF with CPP-ACP = 4,164.0 ± 1,019.3; 5% NaF with TCP = 4,183.2 ± 1,527.2; p = .999), but all of them differed from the negative control group (6,757.8 ± 2,274.7; p < .001). Lesion depth was lower in varnishes groups compared to negative control (% reduction: 5% NaF = 41.8%, 5% NaF with CPP-ACP = 38.8%, and 5% NaF with TCP = 36.3%; p < .001). Similar Ca, P, and Ca/P ratio percentages among groups and F was not detected after the treatments. All fluoride varnishes showed potential to enhance remineralization of early caries lesions in primary teeth.

INTRODUCTION: This study aimed to evaluate (1) the effect of irrigating solutions and intracanal medicaments on the release of transforming growth factor beta 1 (TGF-ß1) and vascular endothelial growth factor (VEGF) from cervical root dentin and (2) the effect of associating triple antibiotic paste (TAP) and calcium hydroxide paste (CH) with 2% chlorhexidine (CHX) on TGF-ß1 release. METHODS: First, 119 specimens from roots (cervical thirds) were obtained and were distributed into 5 groups: 2% CHX, 2.5% sodium hypochlorite, TAP, CH, and 10% EDTA by each growth factor (TGF-ß1 [n = 8] and VEGF [n = 8]). Then, specimens were distributed as follows (n = 13): TAP + 2% CHX, CH + 2% CHX, and 10% EDTA and treated with irrigating solutions and intracanal medicaments. After the treatments, the specimens were immersed in 10% EDTA (20 minutes), and the solution was analyzed using the enzyme-linked immunosorbent assay. The data were submitted to normality, homogeneity of variance, and Mann-Whitney tests (P < .05). RESULTS: Significant differences were found between the irrigating solutions (P < .05) and intracanal medicaments for TGF-ß1 (P < .05). No VEGF release was detected for any group. Our results showed no significant differences among the TAP + 2% CHX and EDTA groups for TGF-ß1 but a significant difference between CH + 2% CHX and the other groups (P < .05). CONCLUSIONS: The use of 2% CHX as the irrigating solution, CH as the intracanal medicament, and 10% EDTA as the final irrigation provides higher TGF-ß1 release from the cervical root dentin, whereas VEGF was not detected. Moreover, TAP and 2% CHX with 10% EDTA as the final irrigation resulted in greater TGF-ß1 release from cervical root dentin than CH + 2% CHX.

PURPOSE: To evaluate the association of dental caries with behavioural, socioeconomic and cultural factors; and Streptococcus mutans (SM) levels in the saliva and oral hygiene index of children aged 4 and 6 years old placed in an oral health programme. MATERIALS AND METHODS: This study was an analytic cross-sectional oral health survey conducted over a 9-month period. A total of 466 children aged 4 and 6 years old were included for evaluation of SM levels in saliva, simplified oral hygiene index and dental caries activity. RESULTS: High SM levels were associated with dmft index, toothbrushing without parental assistance, deficient oral hygiene and ingestion of sweet foods. Deficient oral hygiene was found in children aged 4 years old and with three or more siblings. Dental caries was associated with low family income, deficient oral hygiene, sucrose ingestion by children younger than three years old, bottle-feeding habit and low parental compliance. CONCLUSIONS: High SM levels in saliva, deficient oral hygiene and high frequency of sucrose ingestion had association with dental caries in children. Cultural, socioeconomic and behaviour factors indirectly influenced the onset of dental caries.

This study evaluated gloss and color changes of esthetic restorative materials subjected to different acidic beverages. Specimens of resin composites (Z350XT (Z350), IPS Empress Direct (ED), Charisma Diamond (CD)) were prepared and the initial surface gloss and color (ΔE) were measured (n=10). Then, the specimens were immersed in 4 mL of each of the different beverages (cranberry juice; Coca-Cola; coffee or artificial saliva) during 15 min, 3x/day for 14 days and new gloss and color readings were obtained. Color change was evaluated with the ΔE formula and gloss change values were obtained by the formula: (final gloss - initial gloss). Data was submitted to two-way ANOVA followed by Tukey's post hoc test (a=0.05). CD showed the lowest color change among resin composites. The highest ΔE values were obtained after immersion in coffee and cranberry juice. Coffee promoted the highest gloss change (worst gloss retention), followed by cranberry juice, Coca-cola and artificial saliva (p<0.05). The type of beverage significantly influenced the gloss of resin composites. Coca-cola reduced gloss of the three resin composites in a similar manner. Coffee affected the ED gloss more than that of Z350 and CD, while cranberry juice affected Z350 more than ED and CD. Saliva had a more pronounced effect on the gloss retention of CD than ED. The beverages used in this study influenced the optical surface properties of the composites studied.

Resumo O objetivo foi avaliar as alterações de brilho e cor de materiais restauradores estéticos submetidos ao armazenamento em diferentes bebidas ácidas. Foram preparados espécimes de compósitos de resina (Z350XT (Z350), IPS Empress Direct (ED), Charisma Diamond (CD) e o brilho e a cor (ΔE) de superfície mensurados (n=10). Em seguida, os espécimes foram imersos em 4 mL de cada uma das diferentes bebidas (suco cranberry, Coca-Cola, café, ou saliva artificial) durante 15 min, 3x/dia durante 14 dias e o brilho e a cor novamente mensurados. A alteração de cor foi avaliada pela fórmula ΔE e os valores de alteração de brilho foram obtidos pela fórmula: (brilho final - brilho inicial). Os dados foram submetidos à ANOVA e Tukey (a = 0,05). CD apresentou menor alteração de cor quando comparado aos demais compósitos e maiores valores de ΔE foram obtidos após imersão em suco de café e cranberry. O café promoveu maior perda de brilho, seguido pelo suco cranberry, Coca-cola e saliva artificial (p <0,05). O tipo de bebida influenciou significativamente no brilho dos compósitos resinosos. Coca-cola reduziu o brilho dos três compósitos de maneira similar. O café promoveu maior alteração do brilho do ED, quando comparado ao Z350 e CD, enquanto o suco de cranberry afetou mais o Z350 do que o ED e o CD. A saliva teve efeito mais significativo na perda do brilho do CD do que o ED. As bebidas utilizadas neste estudo influenciaram as propriedades ópticas de superfície dos compósitos.

Aim: To evaluate the performance of three digital radiographic systems in the analysis of root canal filling quality using different intracanal materials for primary teeth. Methods: Twenty-five bovine teeth were divided into 5 groups: Calen® combined with iodoform; Calen® combined with zinc oxide; zinc oxide and eugenol; UltraCal®XS, and 2% chlorhexidine combined with Ca(OH)2 + zinc oxide. Periapical radiographs were obtained with the VistaScan, Express, and SnapShot systems. The quality of the images was evaluated objectively (radiopacity) and subjectively (apical sealing and filling homogeneity). As the reference standard, the teeth were scanned with a micro-CT device. Results: Radiopacity differed among the radiographic systems and materials tested. In general, the greatest difference was observed between the Express and VistaScan systems; Calen® combined with iodoform resulted in the highest radiopacity. The radiographic systems did not differ in terms of homogeneity. However, Calen® combined with iodoform differed from the other materials and exhibited the best results. Regarding apical sealing, the SnapShot system and Calen® combined with zinc oxide provided the best results. Conclusion: Direct digital systems show better performance in evaluating the quality of endodontic treatment in primary teeth and should be preferred for this purpose

OBJECTIVE: To investigate the effects of conditioning solutions containing ferric chloride (FeCl3) on resin-dentin bond strength; on protection of dentin collagen against enzymatic degradation and on cathepsin-K (CT-K) activity. METHODS: Conditioning solutions were prepared combining citric acid (CA) and anhydrous ferric chloride (FeCl3) in different concentrations. The solutions were applied to etch flat dentin surfaces followed by bonding with adhesive resin. Phosphoric acid (PA) gel etchant was used as control. The microtensile bond strength (µTBS) was tested after 24h of storage in water and after 9 months of storage in phosphate buffer saline. Dentin slabs were demineralized in 0.5M EDTA, pre-treated or not with FeCl3 and incubated with CT-K. The collagenase activity on dentin collagen matrix was examined and characterized by SEM. Additional demineralized dentin slabs were treated with the conditioning solutions, and the amount of Fe bound to collagen was determined by EDX. The activity of CT-K in the presence of FeCl3 was monitored fluorimetrically. Data were analyzed by ANOVA followed by post-hoc tests as required (α=5%). RESULTS: Slightly higher bond strengths were obtained when dentin was conditioned with 5% CA/0.6% FeCl3 and 5% CA-1.8%FeCl3 regardless of storage time. Bond strengths reduced significantly for all tested conditioners after 9 months of storage. Treating dentin with 1.8% FeCl3 was effective to preserve the structure of collagen against CT-K. EDX analysis revealed binding of Fe-ions to dentin collagen after 15s immersion of demineralized dentin slabs into FeCl3 solutions. FeCl3 at concentration of 0.08% was able to suppress CT-K activity. SIGNIFICANCE: This study shows that FeCl3 binds to collagen and offers protection against Cat-K degradation. Mixed solutions of CA and FeCl3 may be used as alternative to PA to etch dentin in resin-dentin bonding with the benefits of preventing collagen degradation.

STATEMENT OF PROBLEM: Glass fiber post cementation procedures have undergone significant development. Relining the post with composite resin is a technique that aims to reduce resin cement thickness and consequently problems inherent to polymerization. Evidence is sparse regarding the efficacy of bonding procedures at increasing depths (from cervical to apical) using different adhesive cementation techniques. PURPOSE: The purpose of this in vitro study was to evaluate the push-out bond strength (PBS) of composite resin relined glass fiber posts cemented to bovine root dentin using different adhesive cementation protocols. MATERIAL AND METHODS: Eighteen bovine teeth (n=6) were embedded in polystyrene resin blocks, and the crowns were sectioned leaving a root portion of 20 mm in length. Root canals were prepared using rotary instruments provided by the post manufacturer (Whitepost DC #1), resulting in a uniform root canal preparation. The root canals were lubricated with a water-soluble glycerin gel. Silane (Prosil) was applied and the posts relined with a microhybrid composite resin (Filtek Z100) to conform to the root canal anatomy. Three adhesive cementation protocols were evaluated: a 3-step etch-and-rinse adhesive system (Adper Scotchbond Multi Purpose) in combination with a dual polymerizing resin cement (RelyX ARC); a universal adhesive system (Scotchbond Universal) associated with a dual polymerizing resin cement (RelyX Ultimate); and a self-adhesive dual polymerizing resin cement (RelyX Unicem 2). The roots were sectioned, resulting in four 2-mm segments at 4 different depths (cervical to apical) and evaluated by the PBS test, using a universal testing machine (Instron 4411) at 0.5 mm/min, until failure. Interfaces were evaluated by scanning electron microscopy, and failures were classified as cohesive failure in composite resin, cohesive failure in cement, cohesive failure in root dentin, adhesive failure, or mixed. Data were analyzed by 2-way split-plot ANOVA and the Tukey post hoc test (α=.05). RESULTS: No statistical differences were found for PBS when different cementation systems were compared (P=.253). Increased depths resulted in reduced PBS values for all cementation protocols evaluated (P<.001). CONCLUSIONS: Regardless the cementation system used, the lowest PBS values were at the apical region of resin relined glass fiber posts bonded to root dentin. Furthermore, the highest PBS is observed at the cervical portion of root dentin.

OBJECTIVE: This study evaluated the effect of different filling pastes and cleaning agents on the root dentin of primary teeth using Fourier-transformed Raman spectroscopy (FT-Raman), micro energy-dispersive X-ray fluorescence (µ-EDXRF) and scanning electron microscopic (SEM) analysis. MATERIAL AND METHODS: Eighty roots of primary teeth were endodontically prepared and distributed into 4 groups and filled according to the following filling pastes: Control-no filling (CP), Calen®+zinc oxide (CZ), Calcipex II® (CII), Vitapex® (V). After seven days, filling paste groups were distributed to 4 subgroups according to cleaning agents (n=5): Control-no cleaning (C), Ethanol (E), Tergenform® (T), 35% Phosphoric acid (PA). Then, the roots were sectioned and the dentin root sections were internally evaluated by FT-Raman, µ-EDXRF and SEM. Data was submitted to two-way ANOVA and Tukey tests (α=0.05). RESULTS: Regarding filling pastes, there was no significant difference in organic content. CP provided the lowest calcium values and, calcium/phosphoric ratio (Ca/P), and the highest phosphoric values. For cleaning agents there was no difference in organic content when compared to the C; however, T showed significantly higher calcium and Ca/P than PA. All groups showed similar results for phosphorus. The dentin smear layer was present after use of the cleaning agents, except PA. CONCLUSION: The filling pastes changed the inorganic content, however they did not change the organic content. Cleaning agents did not alter the inorganic and organic content. PA cleaned and opened dentin tubules.

RESUMO

Abstract The quality of the dentin root is the most important factor for restoration resin sealing and drives the outcome of endodontic treatment. Objective This study evaluated the effect of different filling pastes and cleaning agents on the root dentin of primary teeth using Fourier-transformed Raman spectroscopy (FT-Raman), micro energy-dispersive X-ray fluorescence (µ-EDXRF) and scanning electron microscopic (SEM) analysis. Material and Methods Eighty roots of primary teeth were endodontically prepared and distributed into 4 groups and filled according to the following filling pastes: Control-no filling (CP), Calen®+zinc oxide (CZ), Calcipex II® (CII), Vitapex® (V). After seven days, filling paste groups were distributed to 4 subgroups according to cleaning agents (n=5): Control-no cleaning (C), Ethanol (E), Tergenform® (T), 35% Phosphoric acid (PA). Then, the roots were sectioned and the dentin root sections were internally evaluated by FT-Raman, µ-EDXRF and SEM. Data was submitted to two-way ANOVA and Tukey tests (α=0.05). Results Regarding filling pastes, there was no significant difference in organic content. CP provided the lowest calcium values and, calcium/phosphoric ratio (Ca/P), and the highest phosphoric values. For cleaning agents there was no difference in organic content when compared to the C; however, T showed significantly higher calcium and Ca/P than PA. All groups showed similar results for phosphorus. The dentin smear layer was present after use of the cleaning agents, except PA. Conclusion The filling pastes changed the inorganic content, however they did not change the organic content. Cleaning agents did not alter the inorganic and organic content. PA cleaned and opened dentin tubules.

Objetivo: Relatar um caso clínico envolvendo a abordagem cirúrgica/restauradora e medidas preventivas. Relato do caso: Paciente MEMO, 6 anos e 8 meses de idade, feminino, com alto risco/atividade de cárie, apresentando biofilme visível em superfícies lisas livres, lesões de cárie ativa, perda precoce do dente 85, fístula relacionada com a região do dente 75, mudança de cor do dente 61 e perda significativa de dimensão vertical posterior, apresentou-se para tratamento na Clínica de pós-graduação em Odontopediatria da FOP/Unicamp. O plano de tratamento consistiu na abordagem integral focada na promoção da saúde como desfecho principal, ou seja, mudança de comportamento em relação à saúde bucal, e como desfecho secundário, a abordagem cirúrgica/restauradora. Instrução de higiene bucal foi realizada em todas as sessões. O manejo do comportamento foi conduzido por meio da evidenciação e controle de biofilme, profilaxia e adequação do meio bucal seguido da extração do dente 75, selamento de fossas e fissuras dos dentes 36, 46 e 16 e restaurações no 55 e 65. Mantenedor de espaço removível inferior foi realizado. Conclusões: Após seis meses pôde-se observar ausência de biofilme visível, o índice de placa estava controlado e não havia evidência de novas lesões de cárie. Pode-se concluir que o sucesso obtido neste caso destaca a importância da abordagem integral focada na promoção da saúde na clínica odontopediátrica.


Objective: To report a clinic case involving the surgical/restorative approach and preventive actions. Case report: The patient MEMO, aged 6y and 8mo, female, with high caries risk/activity, presenting visible biofilm on free smooth surfaces, active carious lesions, early loss of the 85 tooth, fistula related with the 75 tooth region, color change on 61 tooth and significant loss of posterior vertical dimension has been driven to Pediatric Dentistry Graduate Clinic at FOP/ Unicamp. Treatment plan consisted on integral approach focused as primary outcome on health promotion, behavior change in relation to oral health, and as secondary outcome, the surgical/ restorative approach. Oral hygiene instruction was conducted in all sessions. The behavior management was conducted by evidence and control of visible dental plaque, prophylaxis and oral environment adequation; extraction of 75 tooth, pit and fissure sealing of the 36, 46, 16 teeth, and restorations in the 55 and 65. Removable lower space maintainer appliance was made. Conclusions: After 6 months it could be observed that there was not visible biofilm, the IP was controlled. In addition, there was no evidence of new caries lesions. It could be concluded that the success obtained on this clinical case bring about the importance of the integral performance focused on healthy promotion in pediatric dentist clinic.

OBJECTIVES: The aims of this study were to assess long-term impact of tooth eruption stages (ES) on sealant retention on occlusal surfaces previously coated with intermediary bonding layer and to determine caries prevention. MATERIALS AND METHODS: Sixty-five school children were selected (aged 6-10 years), with four non-carious permanent first molar in different ES (OP (operculum present), ME (marginal edge), CE (completely erupted)). Split-mouth and single-blind study design was used. The teeth (260) were randomly selected according to treatment (sealant/technique): F (Fluroshield), H (Helioseal Clear Chroma), SF (Single Bond + F), EH (Excite + H). Sealant retention, marginal integrity, discoloration, and caries prevention were assessed after 6, 12, 18, and 24 months by calibrated examiner (Spearman = 0.91) using visual inspection. Data were submitted to the Cox proportional hazard model (survival analysis) and Likelihood ratio χ 2 test (correlation), p ≤ 0.05. RESULTS: At baseline, ES was 20 % in OP, 54 % in ME, and 26 % in CE. There was no significant difference on sealant retention between the treatments (p = 0.2774). However, significant differences were found regarding the ES on sealant retention (p = 0.0041). The CE stage showed the highest retention survival rate during the 24 months. The overall sealant prevention average was found to be about 99.4 % and showed no difference between the groups. CONCLUSIONS: Eruption stages affect sealant retention irregardless of the intermediate layer and type of sealant. However, there was caries prevention on tooth occlusal surfaces after 24 months, regardless of treatment. CLINICAL RELEVANCE: Sealing is recommended to prevent occlusal caries of newly erupted teeth in high-caries-risk patients. However, its application is critical on moisture limited control surfaces independently of sealer material and technique.

OBJECTIVES: To evaluate physical-chemical properties of experimental diacetate chlorhexidine (CHX)-added resin blends. METHODS: Blends were formulated: G1)TEGDMA; G2)TEGDMA/0.1%CHX; G3)TEGDMA/0.2%CHX; G4)TEGDMA/UDMA; G5)TEGDMA/UDMA/0.1%CHX; G6)TEGDMA/UDMA/0.2%CHX; G7)TEGDMA/BisEMA, G8)TEGDMA/BisEMA/0.1%CHX; G9)TEGDMA/BisEMA/0.2%CHX. Icon(®) was the control group. For sorption/solubility (SS), cylindrical specimens (n=5) were prepared and their weight obtained. The specimens were immersed in deionized water for 7days at 37°C and their weight was verified again. SS were calculated using accepted formulas. For softening, cylindrical specimens (n=10) were prepared and initial Knoop hardness number (KHN) obtained. The specimens were immersed in absolute ethanol for 24h at 37°C and final KHN accomplished. Softening values were calculated by KHN reduction percentage. For elastic modulus (EM) and flexural strength (FS) bar specimens were prepared (n=10) and values obtained with a universal device (three point, 5mm distance, 0.5mm/min, load of 50N). The data was analyzed using one-way ANOVA and Tukey test (α=5%). RESULTS: TEGDMA/BisEMA blends and Icon(®) showed the lowest sorption from blends (p>0.05), and Icon(®) was the most soluble material (p<0.01). TEGDMA/UDMA/0.1%CHX showed the highest softening, similar to Icon(®) (p>0.05). For EM, all blends were different than Icon(®) (p<0.01). For FS, TEGDMA blends were similar to Icon(®), showing the lowest averages (p>0.05). CONCLUSIONS: Monomers chemical characteristics influenced the physical-chemical properties of experimental blends more than CHX. Between the blends tested, UDMA blends presented satisfactory results for assays evaluated. CLINICAL SIGNIFICANCE: Infiltrants CHX-added could arrest and reinforce initial caries lesions, and the antimicrobial effect could prevent new lesions in sound enamel adjacent to the infiltrated area.

OBJECTIVES: The aim of the study was to evaluate antimicrobial inhibition zone, degree of conversion (DC) and Knoop hardness (KH) of experimental infiltrants. Experimental low viscosity monomer blends were prepared and chlorhexidine diacetate salt (CHX) (0.1% or 0.2%) was added comprising the groups: G1) TEGDMA; G2) TEGDMA/0.1CHX; G3) TEGDMA/0.2CHX; G4) TEGDMA/UDMA; G5) TEGDMA/UDMA/0.1CHX; G6) TEGDMA/UDMA/0.2CHX; G7) TEGDMA/BISEMA; G8) TEGDMA/BISEMA/0.1CHX; G9) TEGDMA/BISEMA/0.2CHX. Icon® was used as control group. METHODS: Specimens of resin blends were made (n=9) to accomplished DC and KH. Pour plate was accomplished to evaluate antimicrobial groups' activity against Streptococcus mutans (SM) and Lactobacillus acidophilus (LA). Data obtained were submitted to two-way ANOVA and Tukey tests for blends comparisons and Dunnett's test for comparisons between experimental infiltrants and Icon® (p<0.05). RESULTS: In relation to antimicrobial effect, uncured blends showed higher antibacterial activity than cured ones for the most of blends. After polymerization, G5 showed the highest inhibition zone against SM and, G3 and G6 against LA. Concerning KH, TEGDMA/UDMA-based blends showed the highest values of KH number and it was influenced by monomeric base, regardless CHX concentration. DC was not affected by monomer blend composition, neither for CHX concentration. The antimicrobial activity was affected by monomeric base, CHX concentration and polymerization. Experimental infiltrants presented similar or higher performance than Icon® for the properties evaluated. SIGNIFICANCE: White spot lesion infiltration with low viscosity monomer blends (infiltrants) is an alternative to stop initial caries lesions progression. The incorporation of an antimicrobial agent as chlorhexidine diacetate salt in infiltrants composition could enhance the performance of these materials.

PURPOSE: To evaluate the penetration ratio of filled and unfilled resin-based sealants on different enamel substrates and pit and fissure morphologies. METHODS: Forty-eight occlusal enamel blocks obtained from impacted human third molars were randomly divided (n equals eight) according to enamel substrates (sound; caries-like lesion; caries-like lesion plus topical fluoride application) and sealant material (FluroShield; Helioseal Clear Chroma). Sealants were applied on the enamel surface. The specimens were stored in 100 percent humidity for 24 hours at 37 degrees Celsius, sectioned in a buccal-lingual direction (at approximately 50 µm), and examined to determine the sealant penetration ratio (b x 100/a; a equals total fissure length and b equals sealant penetration length) and pit and fissure morphology (V-, U-, or Y-shaped). Statistical analysis was performed using Friedman and Kruskal-Wallis tests (P<0.05). RESULTS: Enamel substrate and sealant material did not affect the sealant penetration ratio, with no interactions between these factors. Moreover, the morphology significantly affected the sealant penetration, with the "Y"-shaped fissures presenting the lowest sealant penetration ratio compared to "U-"shaped (P=0.0001) and "V-" shaped fissures (P=0.0018). CONCLUSIONS: Pit and fissure morphology was a critical factor on sealant's penetration capacity; however, enamel substrate and sealant type did not affect sealant's penetration ratio.

RESUMO

Este caso discutiu a abordagem multidisciplinar da reabilitação oral estética e funcional de uma criança com alto risco à cárie com perda prematurade dentes decíduos devido a complicações do tratamento endodôntico associado a cárie dentária generalizada. O paciente foi diagnosticado com mordida aberta anterior e deglutição atípica. O mesmo apresentou ansiedade e baixa auto-estima devido ao comprometimento estético. Combinação de procedimentos clínicos direcionados, com ênfase e foco no cuidado bucal domiciliar e o envolvimento ativo dos pais fez parte do processo de tratamento integral do paciente. Este foi examinado mensalmente por um odontopediatria, o qual orientou sobre hábitos alimentares, exposição a fluoretos, presença de biofilme, prevalência de cárie e má oclusão. Evidente melhora foi observada na função mastigatória,estética facial e comportamento psicológico aos 12 meses de acompanhamento clínico do caso.


This case discusses a multi-disciplinary approach to oral functional and esthetic rehabilitation on a high-caries-risk child with prematurely lost primary teeth due to endodontic complications associatedwith wide-spread dental caries. The patient was diagnosed with an anterior open bite and atypical swallowing. He exhibited anxiety and low selfesteemdue to esthetic impairment. A combination of targeted clinical procedures, a focus on oral home care and active parental involvement was integral to the treatment process. The patient was examined monthly by a pediatric dentist regarding dietary habits, fluoride exposure, biofilm= presence, caries prevalence, and malocclusion. Improvement was evident in patient’s masticatory function, facial esthetics, and psychological behavior at a 12-month follow-up evaluation.
